Outreach Advocate
The Outreach Advocate will provide clients direct support through at least weekly visits and group activities. In addition, this position serves as a primary advocate for the client with the Department of Children and Families, the youth’s school, the legal and other systems with whom the youth interacts. The Outreach Advocate will insure that all necessary services are provided for the youth and his/her family and will identify resources and complete referrals as necessary.
Duties Include
- Meetings with DCF staff, client and clients family, and other involved providers to develop Individual Service Plan;
- Meet with each client at least once per week, one-to-one, to discuss progress and future goals;
- Conduct weekly home visits in order to provide family support;
- Make referrals to other service providers as needed
- Meet with collaterals to support the client and client’s family;
- Provide general client advocacy (i.e.: courts, school, job sites…)
- Plan and run educational and recreational trips and other activities;
- Provide crisis intervention 24 hours/7 days a week;
- Do monthly progress reports for each client and maintain files;
- Prepare and submit all required reporting data;
- Provide transportation as necessary;
- Provide office and drop-in-center coverage;
- Supervise clients while in the center/building;
- Attend supervision with the Executive Director;
- Attend all staff meetings;
- Attend any trainings scheduled as part of staff development;
- Other duties as assigned by the Executive Director
Requirements
- Experience working with inner city youth is necessary as are excellent communication and problem solving skills.
- This position requires a Bachelor’s degree in human services or a related field or at least 3 years of direct service experience.

Salary Range
$25,000-28,000
